Sexualized violence and misconduct
The form below is provided for members of the VIU community to anonymously inform the institution about a matter concerning sexualized violence or sexual misconduct under the Sexual Misconduct Policy 41.17 and associated Procedure 41.17.001.
Do not use this form if there is an immediate safety concern. If this is an emergency call 911 or Campus Security at 250.740.6600.
Actions available to VIU may be limited with information submitted anonymously. Should you wish to make a formal report of sexualized violence or sexual misconduct contact VIU’s Diversity, Equity, and Human Rights Office at 250.740.6440 or HumanRights@viu.ca.
This anonymous form will be used for the purposes specified and not for any other purposes. Completing this form is optional. You are free to stop filling in the form at any time. Submissions are received by the Office of the Associate Vice-President, Student Affairs.
